Clinical History

69 year old female with 1.2cm thyroid nodule.

Cytology

Resident Questions

  • <spoiler text="Diagnosis?">
  • Follicular neoplasm (Hurthle cell type). </spoiler>

Cytologic Features

  • Pure Hurthle cell population
  • Prominent nucleolus
  • Pseudopsammoma bodies (Basophlic structures thought to be colloid, with concentric lamellae)
  • Shows either small cell dysplasia (smaller cells with high N/C ratio) or large cell dysplasia (large cells with at least 2x variability in nuclear size)
  • No lymphocytes
